Description

This specialty rotation is focused on the student who believes that anesthesiology is a likely career choice, has completed the three week rotation (ANES 7181) at UMMC, and desires additional experience at this busy, comprehensive community/county hospital. The student will supplement his core knowledge from the prior experience at a major academic tertiary care institution (UMMC, Fairview) by participating in the perioperative care of patients in a private practice setting. Students will learn more advanced concepts during care of all types of surgical patients, and see a "community perspective" from accomplished practitioners. The experience will build on the basics learned in rotation 7181, and include greater utilization of manual skills such as starting i.v. catheters, endotracheal intubation, and a greater understanding of physiology and interpretation of data from multiple simultaneous monitors. 

Daily student/staff interactions include discussion of general anesthesia, perioperative cardiovascular physiology/hemodynamics, clinical use of intravenous and inhalational anesthetics, neuromuscular blocking drugs, pulmonary physiology, the generation and treatment of pain, and care of patients in the PACU. The student will spend the majority of his/her time in the operating room. This busy environment is an excellent location for students with an anesthesia focus to learn more advanced concepts and skills. Night or weekend call is nil. This is a three week rotation.

Goals and Objectives

  • Understanding how pre-operative assessment impacts the anesthesia care plan.
  • Differences of intravenous vs. inhalation induction techniques.
  • Vocalize important concepts in the care of the unconscious patient.
  • Understand the unique challenges of ambulatory and outpatient surgery.
  • Participation in the operative positioning and homeostasis of all surgical patient. 
  • Management of acute respiratory and cardiovascular derangements in the unconscious patient.
  • Training in basic skills of entotracheal intubation, airway management, and use of special devices like laryngeal mask airways (LMA).
  • Medical care in post-operative care unit, SICU, and pain clinic. 
  • The opportunity to observe and learn from the anesthesiologists in private practice of this specialty.

Evaluation

The final grade will be based on the composite of individual evaluations by staff members with whom the student was most frequently associated.  This will be collated and coordinated by the Course Director.

Prerequisites

ANES 7181. Each three-week rotation assumes a basic understanding of anesthesia fundamentals and progresses according to each student's performance.
